Fill out this form completely to open a new ticket with the helpdesk. You will receive notification by email when we have started processing this ticket (please enter a correct EMAIL address).

It is important that you choose the correct PRIORITY LEVEL, so that we may effectively process all requests. If your problem is keeping you from doing your job, you will want to select "HIGH" priority. If your problem is interfering with your job, but not preventing you from doing your job, select "MEDIUM" priority. If the ability to do your job is not affected, please select "LOW" priority.

If your problem deals with a specific machine, please enter either the EQUIPMENT NAME or BARCODE number of that machine. Enter only ONE piece of equipment per ticket.
